About ECHF

A community-based approach to lasting health.

We work alongside families, clinics, faith communities, and local agencies to make preventive health real, practical, and reachable.

Our Purpose

Why ECHF exists.

Essential Community Health Foundation exists to improve community health, advance health equity, and expand access to preventive health education and wellness resources for underserved and vulnerable communities.

The foundation focuses on helping individuals and families better understand, prevent, and manage chronic health conditions through education, outreach, wellness programs, and connection to community resources.

Mission

"To advance health equity by providing community-based health education, chronic disease prevention programs, wellness resources, and health-navigation support that improve access to preventive care and promote healthier communities."

Who We Serve

Neighbors facing real barriers to care.

ECHF works with individuals and families whose health is shaped by barriers outside the exam room — from cost and transportation to language, literacy, and the sheer complexity of the system.

  • Income and cost of care
  • Access and transportation
  • Health literacy
  • Chronic disease burden
  • Language and cultural fit
  • Lack of preventive care history

How We Work

Partnership-based, always.

ECHF believes lasting health change happens through trusted local partners. We co-design programs with community organizations, healthcare partners, schools, churches, local agencies, and public health stakeholders — meeting people where they already gather.
